Real staples! kathunk, kathunk...

Anyway, they are real staples, the heavy-duty staple gun type, with the pointy bits snipped off with cutters. Cut just after the curve, so instead of a U you have a u (you get the idea).

The "incision" and "stich holes" are just black eyeliner then a deep reddish eyeliner on top, gives the illusion of a deep, meaty wound. Then a dark powder around the dots to create the illusion of the bumps. Then a spot of eyelash adhesive on each "dot", then glue the staples on when tacky.

It holds up well for a photoshoot, but during the night, facial expressions that affect the forehead made a couple pop loose, and by the end of the night's show, he'd sweated off most of the rest, but since it was a stage show, it didn't matter, it was mainly to show off in the poster.
